Things You Need To Know Before Making Your Appointment:

1. Process your $75 payment with our third party partner InstaMed. Setting up an account is not required, you can pay as a guest. Enter your full name, date of birth, amount, and email address to receive a receipt from Parkside Family Medicine.
2. Email the following documentation to: [email protected].
   – PA Photo ID
   – Current Patient Card

PA MMJ Cannabis Industry Initial Certification into the Program

Each patient will FIRST need to create a profile with the DOH.

1. Please register with the Department of Health online using a laptop or desktop computer under Adult Patient Registration at Support Login (custhelp.com) Under Patients CLICK Adult patient registration.
2. Enter in all information on your PA ID EXACTLY as it appears using all CAPS.
3. *Save your login credentials to pay your $50 state fee through your Make Payment tab AFTER your physician appointment.
